Appeals: 180 days from EOB date (first level)
EFT and ERA enrollment
Two options: Zelis Payments Network (call 1-888-679-0665), which sends ACH+ or virtual card payments and 835 ERAs to your clearinghouse or PMS; or ePayment Center, ACH payments with a portal for PDF and 835 ERAs. The 2024 newsletter lists ePayment Center at both 1-855-774-4392 and 1-844-815-9763.
Credentialing and network enrollment

Since April 2025 MetLife credentials dentists through SKYGEN Dental Hub, a multi-payer platform that centralizes primary-source verification and aligns recredentialing dates, replacing the separate 3-year recredentialing packet. No dedicated SKYGEN phone number is given.
Plan clauses to check during verification
Missing tooth clause
Whether a missing tooth clause applies depends on the group plan. Ask whether the plan excludes replacing teeth lost before coverage began, and note the answer before you present a bridge or implant. See how to spot the missing tooth clause.
Waiting periods
Waiting periods are set by the group plan. Confirm the waiting period for basic and major services and the patient's effective date.
Downgrades (alternate benefit)
FEDVIP plan: certain listed services are subject to dental review and the alternate benefit.
Frequency limits
Frequency limits vary by plan. Confirm cleanings, exams, bitewings, full-mouth X-rays and fluoride limits, and the date of the last service billed.
Verification checklist for MetLife Dental
On the Dental Office Service Line IVR, enter patient SSN or employee ID (in-network) or ZIP plus Tax ID (out-of-network), then request a benefits or claim-history fax.
Register the whole office team on dentalprovider.metlife.com; MetLife says self-service access is available to providers and staff.
Try the MetDental.com chatbot for quick coverage lookups before calling.
For COB (E7) requests, send the relationship to patient, other policyholder's name, DOB, coverage effective date, plan type and any custody order up front.
First-level appeals are due within 180 days of the EOB; a second-level appeal is allowed within 60 days of the first denial.
Because the newsletter gives two ePayment Center numbers, confirm the current EFT/ERA enrollment path with the Dental Office Service Line first.
Record the representative's name, the call reference number and the date for every phone verification.
