Waiting periods are set by the group plan. Confirm the waiting period for basic and major services and the patient's effective date.

Downgrades (alternate benefit)

Handbook: where alternative treatments exist, benefits are for the least costly professionally acceptable treatment; the difference for a costlier treatment is collectable from the patient.

Frequency limits

Frequency limits vary by plan. Confirm cleanings, exams, bitewings, full-mouth X-rays and fluoride limits, and the date of the last service billed.
