Timely filing limits

  • Original claims: 1 year from date of service, or from the primary carrier's payment date under coordination of benefits, whichever is later.

  • Appeals: 30 days for each step: re-review from the claim determination; internal appeal from the determination or re-review decision; external appeal from the internal appeal decision.

Dentists and offices are recredentialed at least every 3 years; DentalXChange emails when recredentialing is due. New Premier and PPO applications typically take 18 to 21 business days.

Plan clauses to check during verification

Missing tooth clause

Delta Dental of New Jersey states it has no missing tooth clause. Its Missing Tooth Inclusion covers replacing teeth lost before coverage began, for members age 16 and older on plans with restorative benefits.

Waiting periods

Waiting periods are set by the group plan. Confirm the waiting period for basic and major services and the patient's effective date.

Downgrades (alternate benefit)

Ask whether posterior composites are paid at the amalgam rate and whether crowns downgrade to a base-metal benefit, so the patient estimate is right the first time.

Frequency limits

Handbook processing policy: panoramic images are limited to once every five years; bitewing frequency follows default policy absent group contract language.

  • Re-review, internal appeal and external appeal each have a 30-day window from the prior decision; late appeals are rejected.
