Missing tooth clause

Whether a missing tooth clause applies depends on the group plan. Ask whether the plan excludes replacing teeth lost before coverage began, and note the answer before you present a bridge or implant. See how to spot the missing tooth clause.

Waiting periods

Individual and Family plan: none for diagnostic and preventive; 6 months for basic services; 12 months for major services.

Downgrades (alternate benefit)

Individual and Family plan: a least expensive professionally acceptable treatment clause may apply, such as a partial denture instead of implants; the patient owes the difference.

Frequency limits

Individual and Family plan: exam and 4 bitewing x-rays (age 17 and older) once per 12 months; crown replacement once per 5 years.
