Original claims: 365 days from date of service (Individual and Family plan certificate; group plans not published).
Appeals: 180 calendar days from the original adverse benefit determination or remittance advice (Appeal Request Form, group and individual members). Written decision within 30 days.

Waiting periods
Individual and Family Preferred Prime plan: 6 months for endodontic and periodontal services; 12 months for crowns, bridges, dentures and implants. Group plans may differ.
Downgrades (alternate benefit)
Ask whether posterior composites are paid at the amalgam rate and whether crowns downgrade to a base-metal benefit, so the patient estimate is right the first time.
Frequency limits
Frequency limits vary by plan. Confirm cleanings, exams, bitewings, full-mouth X-rays and fluoride limits, and the date of the last service billed.
