Timely filing limits

Delta Dental of Illinois does not publish one filing deadline for every plan on its provider pages. The limit depends on the employer group or plan, so confirm it on the patient's plan documents or with provider services before you rely on it. EFT and ERA enrollment

Complete the National Direct Deposit Form and fax it to 630-983-4085 (ATTN: Professional Relations), mail it to 111 Shuman Boulevard, Naperville, IL 60563, call 630-718-4990 for secure-email instructions, or upload it in Dentist Connection (your name, then EFT). Offices outside Illinois enroll through their own state's Delta Dental.

To join the PPO network, send the PPO Enrollment Checklist, PPO Agreement, Initial Credentialing Profile and Direct Deposit Form to Professional Relations by fax (630-983-4085) or mail. Requirements include an active state license and adequate professional liability insurance.

Plan clauses to check during verification

Missing tooth clause

Whether a missing tooth clause applies depends on the group plan. Ask whether the plan excludes replacing teeth lost before coverage began, and note the answer before you present a bridge or implant. See how to spot the missing tooth clause.

Waiting periods

Waiting periods are set by the group plan. Confirm the waiting period for basic and major services and the patient's effective date.

Downgrades (alternate benefit)

Ask whether posterior composites are paid at the amalgam rate and whether crowns downgrade to a base-metal benefit, so the patient estimate is right the first time.

Frequency limits

Frequency limits vary by plan. Confirm cleanings, exams, bitewings, full-mouth X-rays and fluoride limits, and the date of the last service billed.

  • For Individual PPO policyholders, use MyDeltaDentalCoversMe (my.deltadentalcoversme.com); registering there also links your Dentist Connection login.

  • Without portal access, use Benefits on Demand fax-back: 800-323-1743 for group members, 855-335-8267 for individual policyholders (24/7). Faxes arrive in about 10 minutes.

  • Keep the verification code the automated system issues; it stays valid for one month if you need a live agent for the same patient.

  • For State of Illinois employee plan patients, use the dedicated IVR at 888-224-0295.

  • The State of Illinois plan is self-funded, so payment waits for the State to release funds, even after approval.

  • Record the representative's name, the call reference number and the date for every phone verification.
