EFT: register on CignaforHCP.com and complete registration validation, then go to Working with Cigna > Enroll in Electronic Funds Transfer (EFT) Options. ERA: ask your EDI vendor or clearinghouse to enroll you for Cigna ERA; each TIN needs its own enrollment.

Call or email with name, office name, address and CAQH ID. Submit credentials through CAQH, One Healthport/Medversant or Cigna's e-onboarding tool, with W-9, dental license, DEA certificate (except orthodontists), specialty certificate if applicable and liability declarations page. Cigna notifies providers when recredentialing is due.

Missing tooth clause
Whether a missing tooth clause applies depends on the group plan. Ask whether the plan excludes replacing teeth lost before coverage began, and note the answer before you present a bridge or implant. See how to spot the missing tooth clause.
Waiting periods
Waiting periods are set by the group plan. Confirm the waiting period for basic and major services and the patient's effective date.
Downgrades (alternate benefit)
DPPO: least expensive professionally accepted treatment applies to posterior composites, crowns/bridges and implant prostheses. DHMO: applies only to crowns and bridges on certain plan codes ("A" in code, legacy "LC", "C" series), not to implant prostheses or material-only differences.
Frequency limits
Frequency limits vary by plan. Confirm cleanings, exams, bitewings, full-mouth X-rays and fluoride limits, and the date of the last service billed.
Verification checklist for Cigna Dental
Check eligibility on CignaforHCP.com > Patients with Patient ID and DOB (or ID/DOB plus name); the same screen serves DPPO and DHMO patients.
By phone, call 800-244-6224, say Provider, enter your Tax ID, request Eligibility, then enter member ID and date of birth.
Cigna Dental Care (DHMO) plans have no deductibles or annual maximums, and general dentists need not send radiographs or preauthorize GP services.
A DHMO plan code with an "A" (e.g. A20V8), starting with "LC" (e.g. LC060) or in the "C" series (e.g. C0608) carries the alternate benefit provision.
Mail paper claims to the right Chattanooga PO Box: DHMO general dentist 188046, DHMO specialist 188045, DPPO 188037.
Mind the filing windows: 90 days for DHMO general dentists, 180 for DHMO specialists, 365 for DPPO.
Record the representative's name, the call reference number and the date for every phone verification.
